Field Trip Reservation Form

Use this form to make a reservations request for School Field Trips. Form submitted is a request. Additional confirmation from A-mazing Acres is needed to complete reservation. Please fill out all requested information. Teachers, we encourage you to download and read the Teacher Field Trip Information document. See a link at the bottom of this page. Payment should be made prior to entering the farm. Only cash or check will be accepted. Cash payment must be in exact amount. Use your mouse or TAB key to move from field to field. Premature clicking of the 'Return' key may cause the form to break. If that happens, depending on your browser, you may be able to continue or, in the worse case, start over.
